Diplomová práce je zaměřena na diagnostiku adrenoleukodystrofie. Jedná se o onemocnění lipidového metabolismu, které je doprovázeno poruchou peroxizomální -oxidace a následnou akumulací VLCFA v plazmě a tkáních. Z pohledu biochemie, diagnostika této nemoci spočívá především ve stanovení VLCFA. Cílem této práce je potvrdit dosavadní biochemickou diagnostiku a zároveň nalézt nové účinné diagnostické markery. Pro analýzu byla nejprve použita tenkovrstvá chromatografie, pomocí které byly analyzované vzorky rozděleny do pěti lipidových tříd: PL, DG, FFS, TG a CE. Následně bylo stanoveno množství jednotlivých mastných kyselin ve zmíněných frakcích plynovou chromatografií. Analýza byla prováděna v plazmě a poté v dílčích lipoproteinech VLDL, LDL a HDL. Pro tuto studii byly použity tři vzorky plazmy od sourozenců z chorvatské rodiny, u nichž se v různé míře projevilo onemocnění adrenoleukodystrofie. Dále bylo zpracováno deset vzorků plazmy od zdravých dobrovolných dárců ve stejném věkovém rozmezí jako pacienti. Na základě stěžejních biochemických diagnostických markerů byla u studovaných sourozenců potvrzena diagnóza adrenoleukodystrofie. Bylo prokázáno, že naměřené hodnoty VLCFA i esenciálních FA korelovaly s věkem chlapců a s ním souvisejícím stupněm onemocnění. Dále bylo zjištěno velmi výrazné zvýšení omega-6 a omega-3 FA u chlapce s nejvíce rozvinutou adrenoleukodystrofií ve frakci PL, přičemž tento nález může mít vliv na poškození membrán, a to především v mozku a nadledvinách. Nebyla nalezena žádná shoda mezi aktivitou lipogenních enzymů a onemocněním. |

The thesis is focused on the diagnosis of adrenoleukodystrophy. This is the disorder of lipid metabolism which is accompanied by the disorder of peroxisomal -oxidation and subsequent accumulation of VLCFA in plasma and tissues. From the perspective of biochemistry, diagnosis of the disease is primarily in determining VLCFA. The aim of this study is to confirm the existing biochemical diagnostics and finding new effective diagnostic markers. For the analysis was first used thin layer chromatography, by which the analyzed samples were divided into five classes of lipid: PL, DG, FFS, TG and CE. Subsequently was determined quantity of individual fatty acids in those fractions by gas chromatography. The analysis was carried out in plasma and then in partial lipoprotein VLDL, LDL and HDL. For this study were used three plasma samples from siblings from Croatian family, who manifested adrenoleukodystrophy disease in varying degrees. There were also processed ten plasma samples from healthy volunteer donors in the same age range as the patients. On the basis of the fundamental biochemical diagnostic markers, the diagnosis of adrenoleukodystrophy has been confirmed on the studied siblings. It has been shown that the measured values of VLCFA and essential FA correlated with the age of boys and the related stage of disease. Furthermore there was found a very significant increase of the omega-6 and the omega-3 FA in the case of the boy with the most developed adrenoleukodystrophy in fraction PL, while this finding may have influence on the damage of membrane, especially in the brain and adrenal glands. There was not found any match between the activity of lipogenic enzymes and the disease. |
